Harlem's Asthma Action Plan

Recently, the state of Illinois passed a law requiring all school districts to request asthma action plans for all students diagnosed with asthma. The law states that the asthma action plan must be requested from the parent/guardian of the student every school year. As a result of your student being diagnosed with asthma we are in need of an asthma action plan that has been developed by your student’s treating physician.
