Required if a parent or guardian seeking to enroll a child needs evidency of residency when a signed lease is unavailable. This includes an expired lease that has automatically become a month to month tenancy. Other documents will also be required to establish residency.
Required if a parent or guardian seeking to enroll a child is living with a District Resident, such as friends or relatives, and they connot provide sufficient proof of residency in their own name.
Original Birth Certificate, Certified Copy or Birth Certificate or Visa when applicable
A certified birth certificate is issued from the county office in the county where the child was born and it will be marked with a county stamp or crimped seal. A certified form from the hospital will not be accepted.
Copies will not be accepted. Our staff will make copies for school records
